We accept virtually all private insurers (except managed care) and accept qualified patients on the Washington State Healthy Options/DSHS program. Please visit our insurance page to see which plans we accept.

Prior to your first office visit, please complete the New Patient Packet to maximize the time you spend with your physician.

If you’re transferring from another practice, please also fill out the Records Transfer Form to authorize the release of pertinent medical information to our office.
